Meet the Hope Hospice of Atlanta Team

Our Hope Hospice care team is coordinated by a physician and registered nurses with the sole purpose of addressing the individual needs of each patient and his /her family.

The interdisciplinary team develops a plan of care specific to the unique issues affecting each patient.

The interdisciplinary team comprises the following professionals:

• Hospice Physician
• Registered Nurses
• Case Managers
• Home Health Aide
• Social Worker
• Hospice Chaplain
• Hospice Volunteer
• Dietician
